Bug Description

1. Login to the system.
2. Click on Survey menu
3. Enter Name/ Disease in the input fields.
4. Click on Reset Filters button.

5. Instead of resetting the results, its filtering the values

Reproduction Steps

1. Enter input values for Survey.
2. Click on reset filter button.
3. It will filter the result, instead of resetting.

Expected Behavior

It should delete the values of the inputs and show all the survies.
