Evaluate where DV_Ordinal (and DV_Scale) make sense

While DV_CODED_TEXT is supported in all places where a data type can be consumed, some places such as CDMConverter.convertConceptMap probably also need the option to consume other data types, specially DV_Ordinal or DV_Scale
