Here is a link to the instructions the SIS office provides for submitting the ​E​form for Current Grad Advisors https://sis.berkeley.edu/sites/default/files/cpp_grad_ja.pdf to submit​ a change of major form​. This form must then be ​​approved​ by the current major advisor and then it will be forwarded to both Tarek and I. I must receive the the completed application through the CITRIS website application portal which can be accessed here: http://citris-uc.org/decse-application/ and Tarek looks at the application and either approves or denies the form​ in calcentral or lets me know and I approve the form.
