The request failed: Failed to fetch!, this is the response when I use the add search engine with the built in nzbindex.com and use the TEST button.

Found the issue.
the search works with this download url: https://nzbindex.com/download/%s.nzb

NZBIndex uses two download urls which both work:

https://nzbindex.com/download?ids=%s
https://nzbindex.com/download/%s.nzb

NZBDonkey uses the first one and nzbindex.com works perfectly with it.
You might have encountered a temporary network error when testing the connection. If the error persists, check the network tab in the browsers dev console (F12) if you see any connection errors:
