Inconsistent recording of "Chest of Gold" #1758

What isn't working? What was supposed to happen?
I've purchased between 1-2 "Chest of Gold" items using "Algari Token of Merit" across 8+ alts, even though it shows the vendor record correctly, the actual looting of the chests seems to intermittently show up.

The result is my Incoming gold count is lower than it should be. I've normally open these immediately when I exchange for them, I've went back and checked alts I don't see them in any bags.
While writing this, I did a test on my rogue, and sure enough the vendor record is there, but no looting is recorded.

No, BugSack+BugGrabber were enabled for "Rogue Test" above, no LUA errors were recorded.
