Apple II: Card Cat fails

I don't immediately understand the issue, but noting that per https://ia601501.us.archive.org/28/items/Apple_Pascal_Operating_System_Reference_Manual_HQ/Apple_Pascal_Operating_System_Reference_Manual_HQ_text.pdf error # 9, S# 0, P# 58, I# 42 is:

• error # 9: System I/O error (XSYIOER): error in attempting to read an operating system segment from disk (FATAL);
• S# 0: current program segment number 0;
• P# 58: procedure number 58 within that segment;
• I# 42: byte 42 within that procedure.

Based on that plus the fact that the image worked in the emulator's never-quite-finished Apple IIGS, which instead uses the IWM, something must be wrong with the Disk II.
