How to make a top level page and assign temlate

I have created a simple addon module (learning it). I am able to create a custom page successfully and template as well. The issue is, created page/pages are sub-pages of index page. I want a separate page.

Current: mysite.com/index.php?m=modulename

Required: mysite.com/modulename.php
