Support importing from legacy points hooks to new hooks API

A class to help with the conversion is included in WordPoints core, but we don't actually perform the import in core yet because there are still a few compatibility issues to be worked out. However, some users may want to go ahead and import. Though we should be sure to make the behavioral changes very clear to them. WordPoints/hooks-api#23 outlines these best, I think.
