Author name template should prompt for ORCID and educational institution

When we process name disambiguations, we need to get more information:

• An ORCID. This will help assign future papers to the correct person.
• The terminal educational institution. This will be used to form their new ID, if needed.

Both of these will be recorded in the name_variants.yaml file under the keys orcid and degree.

This should be added as textboxes to the name correction template.

Are you worried that undergraduate and masters students might not be sure where their terminal degree will be from? I'm pretty sure this was my idea, but I have second thoughts about it. Like I wonder if we should just let people choose any string.

I had that thought, too. But then we need an editorial policy. We might be able to trust that taste will prevent a matt-post-the-awesome; but then, maybe we can't. Not to mention politics and other identity and branding issues, and the fact that people might tire of them, etc. I think this has the potential to be a huge can of worms. The institution you worked under when you first published seems fair enough?

(Limiting or eliminating choices might also be a favor to authors. In high school, when asked at work to choose my userid for my first Linux account, I sweated about it for a weekend and insanely landed on "manimal"—yes, "manimal"—which I had to live with for years. My colleague would have done me a favor by selecting "mjpost" for me).
