Bump version to 0.4.1

- Fix PDF cost estimation (was ~50x overestimated)
- Add intelligent text extraction for PDFs with pypdf
- Add dry run feature for cost preview
- Add configurable token coefficient (1.5x default)
- Reduce logging verbosity for cleaner output
- Add comprehensive test coverage for PDF features
