Fix JSON schema consolidation to properly resolve references

- Remove $id fields from definitions to ensure refs resolve to root
- Fix .yaml refs to use #/definitions/ format
- Replace custom type references with $ref
- Skip invalid 'type: const' entries
- Merge nested definitions into top-level definitions
