This PRs updates references to the "hyperledger" GitHub org in favor of the new "anoncreds" GitHub org. I did not change any of the @ references to the hyperledger org where artifacts from this repo are published. My assumption is that those are not going to change (at least for now). If they do need to change, someone with more knowledge than I needs to make those changes.

Cleaned up a couple of other documentation links as well.
