Turn branch names into links to the actual branches

I like that you mention branches in here, what about turning them into links rather than wrapping with backticks?

What if instead of

• Branch foo/bar/baz

You had:

• Branch foo/bar/baz

We can do that. However there is a possibility of the user deleting the branch, which makes this information useless.

I think that is okay? Thinking mostly of long-lived development branches. These are usually mentioned quite often (unless the branch sentence is only added upon creation and not mention, in that case maybe this is not worth it!)

Maybe we could provide a link to the commit that that branch is at along side of the link to the branch itself would be a good compromise.
