bug: Q CLI sometimes struggles with large todos #3184

• I have searched github.com/aws/amazon-q-developer-cli/issues and there are no duplicates of my issue
• I have run q doctor in the affected terminal session
• I have run q restart and replicated the issue again

Expected behaviour

Q should be able to add items to a large todo.

Actual behaviour

Tool validation failed:
Failed to validate tool parameters: Insertion indices must be unique

Tool validation failed:
Failed to validate tool parameters: Must provide an index for every new task

Tool validation failed:
Failed to validate tool parameters: Index 44 is out of bounds for length 43,

Steps to reproduce

Ask to create a very detailed plan for any complex task.
