Build "Apply for Self-Hosted Projects" page

Current situation

The "Apply for Self-hosted Projects" button currently links to a Google form. This solution is not ideal as it disrupts the user experience by taking them away from the main website.

Proposed Solution

Build a dedicated "Apply for Self-Hosted Projects" page that seamlessly integrates with the existing website design. User will be able to submit project information through a user-friendly form, directly on the website. The design for this page can be found here. This issue relates to #77

Benefits

• Improved User Experience: Users can apply for badging for their self-hosted projects without leaving the website, creating a smoother, more engaging experience.
• Enhanced Consistency: The page would follow the website's design language, leading to a more cohesive user journey.
