Required, unlabeled fields appearing in Description Domain #256

Describe the bug
When a new step is added in the Description Domain (with the "ADD STEP" button), three new, unlabeled fields appear, with two of them required.
To Reproduce
Steps to reproduce the behavior:
- Fill in details for step 1 (change numbering from 0 to 1)
- Click "ADD STEP"
