Parental Waiver File is Missing

I'm in the early stages of planning a 'Bridge event, and was looking for the parental waiver, linked from https://github.com/bridgefoundry/WorkshopCookbook/wiki/Workshop-Planning-Tasks#arrangements-for-children

The link, which goes to here, is no longer working. Could the link be updated to point to the correct location?
