Creating a worktree with the same name causes data loss

Create a session with name "test" then later rename it to "issue-123". Now create a new session "test" and boom your "issue-123" is gone. No warning nothing.

Either don't allow renaming, or rename the worktree folder when renaming the session, or at least check and ask to kill the old worktree folder.
