Create PostgreSQL schema and DB documentation

**Description:**
We want a sql schema as well as setup and documentation for the new DB

**Acceptance criteria:**
- SQL schema in db/schema.sql file
- Setup script that creates the database and applies the schema
- Update documentation in README, etc.
- Update env example
