feat: add Connected Integrations tab to integrations page

- Always show tab interface with Connected, Browse Apps, and Custom MCP tabs
- Connected tab shows existing integrations when available
- Auto-switches to Connected tab when profiles are loaded
- Empty state for Connected tab guides users to other tabs
- Maintains iOS/Android-style tab selector design
- Improves UX by organizing all integration management in one interface
