Updates the module versions to v2.2.0 to add support for OCP 4.3 and terraform 0.12

Default stages to provision IBM Cloud services with Terraform

Due to challenges with the IBM Cloud Operator, update the stages to use the terraform-based modules to provision the services

The issues with the IBM Cloud Operator are listed in the ibmcloud-garage-cloud/planning board. We will be pursuing those items and will revisit the use of the IBM Cloud Operator in the future.
