Update links to https

24 Pull Requests sent me here and I immediately noticed lots of http:// links in the README, so here’s what I did:
* Updated all `http://` links to `https://`
* Checked all the links still work
* Noticed that Phil Sturgeon’s site has moved to `phil.tech`, so updated the URL for that too
