Pre-deployment integrity fixes: remove deprecated packs, add icons, fix docs

- Delete deprecated kai-identity.md and kai-skill-system.md (merged into kai-core-install)
- Delete orphaned kai-da-identity.png icon
- Generate new icons: kai-core-install.png, kai-art-skill.png
- Fix broken anchor in kai-art-skill.md (now points to #-how-pai-packs-work)
- Standardize pack-id format in kai-art-skill.md
- Update install.ts pack list (replace deprecated packs with kai-core-install)
- Update pack count badge from 1 to 6 in README.md
- Add kai-art-skill to Packs/README.md table
- Remove deprecated pack rows from Packs/README.md
- Fix phase count in Bundles/Kai/README.md (two → three phases)
