What happened?

(I do not know what happened, but these are the steps I was taking before the issue occurred.)

I've been using a self hosted instance of LibreChat in Docker on my Synology NAS for a while now without issue.

Today I added a gpt-4.1 assistant on OpenAI platform. This assistant was working fine within LibreChat when I was testing on my Mac in Safari.

I then proceeded to create a home screen app shortcut to my self hosted librechat on my iPhone.
I attempted to use this new assistant on my iPhone but received a blank error when sending the prompt. After a few more attempts to send the prompt, I was logged out of librechat and receive the following error. (The error is present regardless of where I try to sign in.)

Any way to fix this?
And how to turn this ban function off? I self host for myself on my local network. I do not need this.
