add_request_headers in v3alpha1 only accepts an object

Based on the report in https://github.com/emissary-ingress/emissary/issues/4155 the API version `v3alpha1` only accepts an `object`. `string` and `bool` are not accepted anymore and should be removed from the documentation and the examples.
