Multiple options in the same slash command.

I'm having trouble creating more than one option in the same command. I can't understand how it would go. I would appreciate it if you could leave me the code as the options format should be. I have this and it works but with several it doesn't jump or work.

I probably don't have the answer you want, but I've been creating commands like this:
