Basic usage documentation needed

This plugin is listed as a recommended plugin in Django CMS v4.x. But there is no basic usage documentation.

How is this meant to be used?

What are “Form entries”? How are syntax and semantics of “Entry data” and “Header fields”?

When creating an alias with a form using form builder, how works the connection with “Form entries” exactly?

#20 also removed the confusing ability to add "form entries" and their "entry data". Forms submitted by users should show up with redered form entry data in the admin and be self-explanatory.

Still, all contributions to the README (or even docs) are greatly welcome - espcecially if they are based on recent experience with roadblocks etc....
