I honestly think I'm starting to see an issue with the relationships between tokens... I'm starting to think that all the tokens should be related to a grant and that a grant should live for the life of it's tokens.... DOT currently seems to use the refresh token and accesstoken as the representation of the 'grant'... alternatively maybe we need a session, but the relationships between access and refresh tokens seems to be a common thread in the issue queue.
