bug: Revoked ID tokens not deleted on refresh

Describe the bug
When refreshing tokens it appers that old IdTokens are not being removed.

To Reproduce

Start the test/app/idp and test/app/rp, login with the RP the hit refresh token multiple times. The number of access tokens will stay constant, and the number of IdTokens will grow.

Expected behavior
Number of ID tokens stays consistent
