Footnotes link in URL hash

If there is a `#f_xx` hash in the URL, it should automatically open the footnotes. This pattern should also open the footnote in the URL and the server can return the same page it would for the reference to allow that to work. Clicking on a footnote should replace the hash with the syntax to allow the back button to work correctly.
