Right-click menu Source -> Clean Up on project is sometimes unavailable

Right-click menu Source -> Clean Up on project (or source folder) is sometimes unavailable. It is only on a single Java source file that it is always available.

I'm clueless about how that menu activates. Sometimes it just shows Source -> Format grayed out, and sometimes, after many attempts, it will show Source -> Clean Up among other options.
