Add support for outgoing calls in the call hierarchy view

This feature request aims at complementing PR #513 (support for call hierarchy view with incoming calls) by adding support for outgoing calls.

For example, clangd (vers. 20) [now supports outgoing calls](https://github.com/llvm/llvm-project/pull/117673) and could be used in CDT LSP.
