Forms don't work properly with multiple forms on same page

When multiple Charge forms are initialized on the same page, the 1st form in the DOM seems to work fine, but subsequent forms do not — suspect that subsequent forms aren't being initialized properly and/or are conflicting with each other, preventing more than one being usable on a given page/view.
