Table Input Format: Optional Header

It would be great to allow an optional header for table files we are reading in.

This header row can also be used to decide the order of columns (right now we force a specific ordering).

This is a little tricky, because there are a few corner cases:

• No Header
• Cross Product of:
  • [Missing Columns, All Columns, Extra Columns]
  • [In Order, Out of Order]

Since this is an open source project, we don't really do "dibs" on issues.
If you want to make an attempt and submit a PR, you are free to.

That said, spamming multiple issues or PRs is not appreciated and reflects poorly.
