Cleanup OpenAPS spreadsheet and rerun tooling

• Some information in spreadsheet where the UCAs are mentioned in the Loss Scenario text can be removed, at least in the yaml
• Make sure each Loss Scenario has it's own identifier and cell (see 51 and 45)
• Tidy up Loss Scenarios with 'Human' in front (see 670 and after)
• If there are duplicates of Loss Scenarios across different UCAs, or the Loss Scenario is the same as the Controller Constraint for the UCA, it can be removed (see line 73)
• Tidy up line 683 (incomplete instructions isn't human error)
• Split 763 into 2
• Clarify 'dependencies' term (for algorithm or for system)
• Cleanup and add requirements
