`.. autolink-skip::` breaks sometimes

The directive .. autolink-skip:: appears to break under certain conditions when implicit literal blocks are used. See #170 for a demonstration.

As discussed on the PR, this happens when the content is identical in a skipped block and a linked block below that, in which case our matching fails to take that into account. Fixed now!
