Bug: Duplicate items when pasting

Description

Moving items can cause hundreds of duplicates. If copying file to another folder, Files will copy multiple copies to any folder that is visited while the copying process is running. This has also occured when I attempted to drag files into Google Drive (via web browser), but then returned the mouse to the main window because I realized I had the wrong file.

Steps To Reproduce

1. Copy a file
2. Paste into another folder
3. Change directories after pasting

I'm having trouble reproducing this. Did this first start after updating to v4?

I've only encountered this when there's permission issues in the folder you're trying to paste items in, and only happens half of the time
Unfortunately I don't think I'm able to reproduce this exact scenario, but the overall behavior rang a bell for sure

I see a bunch of relevant exceptions in the log file. It would help to know if it's a new issue, and if anyone else is able to reproduce it.

It occurred both before and after the 4.0 update.
