Contributing docs and Github Issues point to incorrect repo #15

Describe the bug
Assuming there's been a name change or two prior to release. Docs currently point to https://github.com/finos/fluxnova-bpm-platform instead of this repo (fluxnova-modeler).
Also noticed the Contributing.md would link to itself but has no guidance. FDC3 has some nice guidance, but not sure if there is a central version you wanted to point at?

- Search repo for https://github.com/finos/fluxnova-bpm-platform
Expected behavior
Should point to https://github.com/finos/fluxnova-modeler
