Improve logging for readability

I'm finding our tracing/logs to be verbose with redundant and duplicated information, which makes finding important details, e.g. the actual log message, difficult when debugging.

Helpers in `utils/trace.rs` should be revised to remove bloat, formatting needs to be improved, and we should figure out a way to reduce target names. Several log messages also need to be improved.
