Migrates the “Vitalparameter” Implementation Guide to the IG Publisher structure and wires it into the GitHub Actions publishing workflow.

Changes:

• Adds a new IG Publisher guide under publisher-guides/Vitalparameter (config, ini, page content, and includes).
• Introduces shared includes for artifact rendering and MS comment data generation.
• Removes now-duplicated text from the legacy guides/Vitalparameter-5/... pages and adds the new IG to the CI workflow matrix.
