I hadn't seen this until about six months ago, but now see it often on my iPad, even with a decent network connection. Other times, I can't reliably reproduce the issue at all.
Sometimes it can be down to vibration, sound, fullscreen etc (#50, #49, #46). But I've seen other unexplained instances.
