Instrument tool calls responses

Client tool calls are only executable on the user's local system so we cannot auto instrument them, but we can emit attributes of calls such as gen_ai.tool.id and gen_ai.tool,name.

Anthropic also supports server tools, these are executable in Claude system, we probably can't do much here, but look into it.
