index: remove duplicate entries

These were unfortunate side effects from the early days where the
GitArtifacts Azure Pipeline did not append any timestamp to the version,
and hence only one file name was used per git-for-windows/git commit
(which prevented multiple snapshots e.g. when updating just the MSYS2
runtime, but not having a commit in Git to justify a new snapshot).
