Duplicate file write permission request

Describe the bug

The first time Copilot CLI wants to write a file in the session, it always asks me twice (presenting the same diff in both requests).

Affected version

No response

• Write hello to file.txt

Expected behavior

Should ask once, not twice

I'm also seeing this in tool invocations as well (when the model generates multiple tool calls in a single turn that are both of the same "shape" (e.g. two calls to npm run) and you approve the first one).

I'm also seeing this in tool invocations as well (when the model generates multiple tool calls in a single turn that are both of the same "shape" (e.g. two calls to npm run) and you approve the first one).

When parallel tool calls are executed, as required, they request permission from the permission service, which internally checks whether approval has previously been granted, and if not, attempts to request permission from the user via the UI. This means that the UI request has already been queued up before the approval has been added to the permission service.
