Open pull requests for user

Brought up in #77, some of the inline suggestions cause problems due to the variables. Let's try another approach, and rework the flow where we open that pull request for the user and have them review it.

It may have to do with the inline suggestion. Should we try and use createPullRequestComment instead of octokit for the suggestion? It should then let us use a response file that we can pass the pagesUrl variable into.
