Open commit explanation in a markdown file

When generating commit explanations from anywhere but the commit details, open the explanation in a markdown file.

This would also:

• enable explanations for the combined diff of multiple commits, which cannot currently be displayed in the commit panel.
• provide a fallback for VSC-based IDEs like Cursor and Windsurf that cannot display explanations in chat
