Include PR #number into changelog entries

When reading the changelog, I sometimes want to look at the PR (and related issue) that introduced it, to learn more about the reasoning behind it. Currently, the changelog only includes a link to the contributor, which is nice. Finding the related PR can be troublesome.

Could you also include a PR reference in the changelog entries, please? 🙏
