What happened?

got this error after a task that had the ai doing multiple steps. not sure if this is part of the issue or not but context left did not change from 100% and ive had the ai do many things that for sure would have used some. The ai did have a few failed tool uses in the same task, but this one happened after it completed some edit work and then it errored right after a readfile.

OK after this error and asking it to complete its task, my context left did change to 68%left, again unsure if this is part of the issue or not, also tool use after worked as well.

I've been getting the same error, I can reproduce it reliably using the new Gemini CLI Security Extension (/security:analyze) does it 100% of the time when it's trying to delete a file that doesn't exist.

I have also seen this error trying to use the GitHub/spec-kit ... again, I think it's always related to trying to edit files in the .gemini folder, which is what both of these extensions are trying to do.
