Update docs for EXTERNAL_BASE

Docs could be updated for the EXTERNAL_BASE env var. When developing locally the url needs to be externally accessible in order to complete the login flow with a PDS. I've been using ngrok and the cmd ngrok http 8080. There's also Cloudfare Tunnels and Tailscale Funnel.
