Support anonymizing of different fields within DICOM.

• Given an option to create patient IDs
• Assign anonymized Patient IDs
• Add these to the crosswalk table
• Create a mapping table for Patients as well.

If the output is a DICOM format, then the output options should show up below, and try to mimic some of the DICOM Cleaner functionalities.
